Job Summary
Coordinates referrals from Nyack Hospital and all other referral sources (i.e., Westchester, New York City and New Jersey Hospitals, Rehab and Sub Acute Facilities, physician's offices, etc.), to develop appropriate home care plans together with the physician, referring organization, patient, and family. Conducts and participates in Quality Assurance Performance Improvement activities appropriate to the Home Health Agency.

General Contacts: Physicians and their office staff, hospital staff, patients, Clinical Managers, Community Health Nurses, Physical, Occupational Therapists, Medical Social Workers, office staff and representatives of official, voluntary, and service organizations. 2. Job Provides Direction to: N/A D. Environment and Demands 1. Environmental Demands: Normal office conditions, heavy workload. Moderate exposure to health hazards. Travel to referral sources by automobile. 2. Essential Physical and Cognitive Demands, Manual Dexterity: Self-directed; ability to work collaboratively as well as independently. High tolerance for stress. B. MISSION/STRATEGIC PLAN: Participates in activities which support the mission of the hospital C. JOB SPECIFIC COMPETENCIES: Demonstrates the required job specific technical/professional knowledge, skills and abilities. * 1. Serves as the liaison between the hospitals and other referral sources and the home care agency to promote collaboration and the continuity of care. Develop and maintain positive relationships with hospital staff, physicians, and community partners. 2. Receives and reviews initial requests for home care services and obtains information necessary to determine patient appropriateness for Home Care. 3. Coordinates and communicates requests for service with the Director of Patient Services and Clinical Managers to ensure that the patient will receive services appropriate to identified level of need. Works with the Director of Patient Services and Clinical Managers to problem solves any unique aspects of care. 4. Documents accurate and comprehensive clinical and demographic patient information in the electronic medical records according to Medicare and other insurance requirements as well as requirements for appropriate PDGM coding and registers and scans patient information into the Home Care information system. 5. Obtains necessary insurance authorizations for service and arranges for prescribed lab work as ordered by the physician. 6. Acts as an educational resource to hospital case managers, physicians, hospital staff and staff at other health care facilities and referral sources regarding the agency’s services. 7. Participates in agency’s required in-service programs. Part II: Employee Performance Standards A. CUSTOMER SERVICE: 1. Demonstrates the ability to create a positive interaction with the customer. 2. Utilizes charge process for supplies/equipment. D. PROJECTS/ASSIGNMENTS: 1. Completes projects/assignments within projected time frames and expectations.
Qualifications

- 2 years clinical experience and 2 years Home Care experience
- MS Excel is a plus
- Registered Nurse New York - New York State Board of Nursing Required or
- Physical Therapists - New York State Board of Physical Therapy Required or
- Occupational Therapists - New York State Board of Occupational Therapy Required or
- Licensed Master Social Worker - New York State Board of Social Work Required or
Valid/ current driver license Upon Hire Required
