Your Role: Community Outreach Specialist
- Client Engagement: Make initial contact with eligible clients by phone after receiving health plan authorization. Offer face-to-face visits to those interested in a home consultation.
- Community-Focused Support: Provide culturally and linguistically sensitive services to meet the unique needs of our community members.
- Program Awareness & Education: Educate potential clients and local organizations about program benefits and services to enhance awareness and participation.
- Client Enrollment: Obtain consent to enroll clients in the program, ensuring smooth handoff to lead care managers.
- Documentation: Keep accurate and timely records in the program’s electronic health system for all client interactions.
- Collaboration: Communicate with your supervisor on referrals, enrollments, and program statuses to ensure consistent, quality service.
- Flexibility: Support other related tasks as needed to contribute to a dynamic and effective care program.
What We're Looking For:
Minimum Requirements:
- Experience: At least one year of experience working with disabled adults or underserved populations.
- Cultural Insight: Lived experience in or familiarity with the community served.
- Detail-Oriented: Strong organizational skills with a knack for detail.
- Communication: Clear and effective communication skills.
- Bilingual: Bilingual English plus Cantonese and/or Mandarin and/or Vietnamese required.
Preferred Qualifications:
- Expanded Experience: More than one year in home health care coordination or working with at-risk communities.
- Industry Insight: Familiarity with health plan organizations or community-based case management.

Physical Requirements
- Communication: Ability to talk and hear during face-to-face communications.
- Mobility: Ability to reach overhead and below the knees, including bending, twisting, pulling, and stooping.
- Coordination: Multi-limb and eye-hand coordination.
- Endurance: Ability to sit at a desk for prolonged periods while working on computers. Ability to move, lift, carry, push, pull, and place objects weighing more than or equal to 40 pounds without assistance.
