Job Summary
We are seeking an experienced and highly motivated Community Outreach Specialist to lead and expand our CALAIM programs throughout Los Angeles County.
Experience working for LAHSA, with LAHSA-funded programs, or within the Los Angeles Homeless Services System is strongly preferred. Candidates with experience in Coordinated Entry, HMIS, housing navigation, street outreach, housing placement, or homeless services are encouraged to apply. The ideal candidate will have 2–4 years of experience working within the homeless services, housing, healthcare, or social services sectors, with a strong understanding of the Los Angeles homeless services and housing landscape.
This is an ideal opportunity for someone who has previously worked with LAHSA or within the Los Angeles homeless services system and has established knowledge of community resources, and housing programs.
Key Responsibilities
- Conduct outreach to hospitals, hospital case managers, social workers, community-based organizations, housing providers, and other referral sources.
- Meet directly with potential members to introduce our programs, explain the benefits and available services, answer questions, and help individuals understand how our programs can support their recovery, housing stability, and transition to the community.
- Develop and maintain strong rapport with hospital discharge planners and case managers.
- Promote our programs within hospitals and throughout the community and clearly communicate our services, eligibility requirements, and referral process.
- Build and maintain relationships with community organizations and service providers serving individuals experiencing homelessness.
- Identify new referral sources, partnerships, and opportunities that can expand our programs and services.
- Use existing knowledge and professional relationships within the Los Angeles housing and homeless services community to generate new opportunities.
- Represent the organization at community meetings, networking events, hospitals, and other outreach settings.
- Collaborate with internal teams to ensure successful referrals, transitions, and housing outcomes.
- Stay informed about available housing resources, supportive services, healthcare programs, and other community resources.
- Track outreach activities, referrals, relationships, and opportunities and provide regular updates to management.
- Bring forward new ideas, partnerships, and opportunities that can strengthen and expand ourprograms.
Qualifications
- 2–4 years of relevant experience in homeless services, housing navigation, healthcare outreach, social services, or a related field.
- Previous experience working with LAHSA or within the Los Angeles homeless services system is strongly preferred.
- Established knowledge of Los Angeles County housing resources and community-based services.
- Experience working directly with hospitals, hospital case managers, social workers, or healthcare organizations.
- Strong outreach, networking, communication, and relationship-building skills.
- Ability to develop meaningful partnerships and turn relationships into referrals and new opportunities.
- Self-motivated, organized, and able to work independently.
- Professional and confident when representing the organization in hospital and community settings.
- Valid California driver's license and reliable transportation required.
Pay: $23.00 - $28.00 per hour
