About the Role
We are seeking a dedicated Care Coordination Manager to join our team and support individuals with Intellectual and Developmental Disabilities (I/DD). In this role, you will serve as the primary point of contact for members, families, and advocates while coordinating healthcare and community services to ensure high-quality, person-centered care.
This is a hybrid position with opportunities available in both Cortland, NY and Binghamton, NY.
Key Responsibilities
- Manage a caseload of individuals with Intellectual and Developmental Disabilities (I/DD)
- Coordinate medical, behavioral health, OPWDD, and community-based services
- Develop, implement, and maintain individualized Life Plans
- Act as a liaison between members, families, advocates, providers, and community resources
- Monitor member progress and ensure services align with individual goals and needs
- Promote person-centered care and positive outcomes
Qualifications
Candidates must meet one of the following requirements:
- Bachelor's Degree and 2 years of relevant experience; OR
- Master's Degree and 1 year of relevant experience; OR
- Registered Nurse (RN) with 2 years of relevant experience
Additional Requirements:
- Valid Driver's License
- Ability to travel locally
- Ability to work a flexible hybrid schedule
