Job Description
The person in this position will work with youth and their families to deliver intensive case management, skills training and medication education primarily in community-based settings, such as homes and schools. This position uses evidenced based treatment, individualized levels of care, and person-centered recovery planning to best serve youth and their families. The person in this position will assist youth and their families with accessing community resources, and provide rehabilitative services and crisis intervention when needed. All services are documented in an electronic health record and provided in collaboration with a multidisciplinary recovery team.
The person in this position will have a passion and aptitude for working with youth who have severe emotional needs. Knowledge of psychotherapeutic medications, case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the behavioral health field is critical for success in the position.
This position necessitates innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning and dual-diagnosis treatment. The person in this position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, and perform other duties as assigned with a positive attitude.
Essential Functions:
Builds and maintains a therapeutic relationship with individuals through engagement
Provides psychosocial rehabilitation along with linking individual with community supports.
Performs assessments with individuals identifying their strengths and needs.
Completes Person Centered Recovery Plans developing goals and objectives with individuals.
Completes documentation in a timely manner.
The Role:
Has initiative and works well in ambiguity.
Engages individuals based on personalized Level of Care.
Completes Assessments within expected time frame (180 days).
Completes Recovery Plans to guide treatment.
Is a member of an interdisciplinary treatment team.
Has access to ongoing Training.
Location: Caldwell County
Hourly Salary Range: $21.46 – $29.71
Range of salary subject to applicable differentials listed below.
Applicable Differentials:
$0.75/hr Bilingual
$1.00/hour LPC-A/LCSW-A
$1.00/hour – 2 years previous experience
$1.00/hour – Advanced Licensure
$0.50/hour – LCDC
