Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our team, part-time. The ideal candidate will play a crucial role in supporting individuals with physical disabilities and traumatic brain injuries remain in the community in their own home. This position requires strong interpersonal skills, and a commitment to empowering clients through effective case management. Retired or semi-retired registered nurses are encouraged to apply. There are no skill-nursing duties required.
Responsibilities
- Conduct comprehensive assessments to identify client needs and develop individualized service plans.
- Collaborate with multidisciplinary teams to coordinate care and services for clients.
- Utilize motivational interviewing techniques to encourage client engagement and progress.
- Maintain accurate documentation of client interactions, progress notes, and service plans.
- Advocate for clients in accessing community resources and services.
- Stay informed about community resources to effectively assist clients.
Experience
- Must be licensed as a registered nurse
- Experience in healthcare service delivery/human services case management pertinent to the following population, severely disabled adults and adults with traumatic brain injuries.
- Have the capability of engaging in and performing a range of activities and interventions to meet the client's needs and advocate on behalf of the client to ensure that their needs are being met.

Pay: $75.00 - $95.00 per hour
