Olivera Health Group is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our integrated healthcare team. This role is ideal for a behavioral health professional who is passionate about improving patient outcomes through collaborative, evidence-based care in a primary care setting.
The LCSW will provide in-person behavioral health services with the flexibility of a hybrid work schedule, partnering closely with primary care providers to deliver comprehensive, patient-centered care.
Responsibilities
- Conduct mental health screenings, psychosocial evaluations, and comprehensive assessments for medically referred and self-referred patients.
- Provide short-term, structured, evidence-based psychotherapy as part of individualized treatment plans.
- Collaborate closely with primary care providers and other members of the healthcare team to improve patient health outcomes.
- Deliver individual and group therapy for patients experiencing mental health and co-occurring disorders.
- Coordinate referrals to specialty behavioral health services, substance use treatment programs, and community resources when appropriate.
- Provide crisis assessment and intervention as needed.
- Advocate for patients by helping address barriers to care through follow-up, care coordination, and collaboration with community partners.
- Maintain accurate, timely clinical documentation in accordance with organizational and regulatory standards.
- Participate in interdisciplinary care planning and quality improvement initiatives.
Qualifications
- Active Illinois Licensed Clinical Social Worker (LCSW) license required.
- Master's degree in Social Work (MSW) from an accredited program.
- Strong interpersonal, verbal, and written communication skills.
- Ability to build rapport with patients from diverse backgrounds.
- Experience in integrated healthcare, medical social work, primary care, or community mental health preferred.
- Experience with evidence-based behavioral health interventions and crisis management is preferred.

Pay: $80,000.00 - $95,000.00 per year
