Adult/Geriatric Nurse Practitioner Job Description
Overview
We are seeking a dedicated and skilled full-time Adult/Geriatric Nurse Practitioner to join our healthcare team. The ideal candidate will possess a strong background in patient care and demonstrate the ability to work collaboratively with physicians and other healthcare professionals. This role involves providing comprehensive care to patients, including assessment, diagnosis, and treatment planning. The Nurse Practitioner will play a vital role in enhancing patient outcomes and ensuring high-quality healthcare delivery.
Duties
- Conduct thorough patient assessments, including Histories and physical examinations with comprehensive documentation of all findings on a daily basis.
- Provide efficient medical management to patients in close collaboration with the supervising physician.
- Develop and implement individualized care plans based on patient needs and preferences.
- Perform discharge planning to ensure smooth transitions for patients leaving the healthcare facility.
- Collaborate with nursing home staff to provide ongoing care for residents.
- Document patient interactions accurately using ICD-10 coding for billing and record-keeping purposes.
- Participate in utilization review processes to assess the necessity of services provided.
Requirements
- Master’s degree in Nursing from an accredited program.
- Current Nurse Practitioner certification and state licensure.
- Excellent Bedside manners a must.
- Strong knowledge of medical management and utilization review practices.
- Excellent communication skills with the ability to work effectively within a multidisciplinary team.
- Proficiency in electronic health record (EHR) systems for documentation purposes.
- Ability to handle multiple tasks efficiently while maintaining attention to detail.
