ABOUT THE POSITION:
Fort Hudson Health System is the region’s only not-for-profit multilevel post-acute provider of community and facility-based care. Our multiple programs and services throughout the region make Fort Hudson an exceptional career opportunity for professionals who value exceptional quality and a passion to serve.
The ideal candidate should be experienced in case management/service coordination, linkages ,and referrals regarding community-based services for youth ages birth to 21 with complex medical needs, mental health diagnoses, developmental delays and/or social needs. Job duties include and are not limited to identifying and determining resources in the community, coordinating, and linking clients to providers, actively intervene and advocate on behalf of clients as needed, provide follow up and transitional services, maintain program documentation as required and adhere to appropriate Medicaid billing requirements. Our Care Managers collaborate with families to identify goals, manage medical care, coordinate services and provide education to help develop strengths and better manage each child's unique needs.
Qualifications:
Qualifying fields of degree: social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services.

Master’s Degree in qualifying field or Bachelor’s degree with 1 year experience, or Associates degree with 2 years experience.
