Position Summary
The Case Manager coordinates the assessment, individualized service planning, and ongoing care coordination for residents of Oyster Bay Senior Campus, ensuring services are delivered in accordance with each resident's needs and applicable facility standards. The Case Manager serves as a primary liaison among residents, families, facility staff, and external service providers.
Key Responsibilities
Responsibilities include but are not limited to the following:
- Conducting and documenting admission, periodic, and change-in-condition assessments, including but not limited to the Individualized Service Plan (ISP) and required reassessments.
- Developing, implementing, and updating individualized service plans in coordination with the resident, family, physician, and interdisciplinary team.
- Coordinating resident care and services with internal departments and external providers, including but not limited to home care agencies, hospice, physicians, and pharmacy.
- Monitoring resident status and ensuring timely follow-up on changes in condition, hospitalizations, returns, and care transitions.
- Maintaining accurate, complete, and current resident records consistent with facility policy.
- Assisting residents and families with care concerns, grievances, and access to community resources and entitlements.
- Participating in interdisciplinary team meetings and quality assurance activities.
Qualifications
Qualifications include but are not limited to the following:
- Associate's or Bachelor's degree in social work, human services, nursing, or a related field, or equivalent relevant experience.
- Experience in an adult care facility, assisted living, long-term care, or comparable setting strongly preferred.
- Working knowledge of adult care and assisted living operations, including but not limited to memory care settings, preferred.
- Strong assessment, documentation, organizational, and communication skills.
- Proficiency with electronic records and standard office software.
- Ability to work collaboratively with residents, families, and staff.
