PMHNP Credentialed Providers: This position is not focused on mental health needs, only physical needs of patients. Primary Care Credentialing is required for this position.
Community Healthcare Partners is seeking a Full-Time Nurse Practitioner (APRN/NP) to provide high-quality primary care services within behavioral health facilities located in Oregon City and SE Portland. Facilities are close in proximity and you can plan on being in one facility each day.
This unique position allows you to focus exclusively on medical care — with no mental health caseload responsibilities — while collaborating alongside an experienced, fully staffed behavioral health team.

What You’ll Do
- Conduct comprehensive physical assessments and review patient histories
- Diagnose and treat acute, chronic, and infectious medical conditions
- Order, interpret, and evaluate laboratory and diagnostic testing
- Prescribe and manage medications and treatment plans
- Educate patients and families on wellness, prevention, and disease management
- Maintain accurate EMR documentation using EClinical Works
- Collaborate closely with clinical and facility teams
- Deliver proactive, compassionate, patient-centered primary care
- Spend time in different facilities through the week.

Qualifications
- Licensed Nurse Practitioner (APRN/NP) in Oregon — required
- Credentialed primary care experience preferred
- Strong clinical assessment and diagnostic skills
- Knowledge of healthcare regulations and compliance standards
- Excellent communication and patient education abilities
- Compassionate, dependable, and team-oriented approach to care
