General Statement: Advanced Pain Care advocates the appropriate and effective management of chronic pain through a multi-disciplinary approach. Job Purpose: The Licensed Clinical Social Worker is responsible for coordinating and implementing appropriate methods of treatment to patients in the behavioral health program.
- Greets patients and introduces self.
- Presents a professional image and customer focus.
- Verifies patient information, medical history, medications, and confirms purpose of visit.
- Conducts pre-surgical evaluations and diagnosis as required for surgical implant procedures
- Provides assessment, diagnosis and individual therapy treatment to patients in the behavioral health program.
- Monitors the effectiveness of interventions.
- Performs routine and crisis counseling/interventions, and suicide risk assessments as needed.
- Provides referrals for services as needed.
- Responsible for the content of education and counseling programs for patients.
- Responsible for appropriate documentation of information in patients' charts (EMR/EHR).
- Secures patient information and maintains patient confidence by completing and safeguarding medical records.
- Maintains safe, secure, and healthy work environment by establishing and following standards and procedures; complying with legal regulations.
- Practices safe habits including but not limited to infection control, lifting procedures, etc.
- Practices teamwork by helping others willingly and communicating in a professional manner
- Coordinates with clinical and other staff members in the provision of services to patients.
- Updates job knowledge by participating in educational opportunities
- Serves and protects the practice by adhering to professional standards, policies and procedures, federal, state, and local requirements.
- Enhances practice reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
- Operates standard office equipment

- Has regular and predictable attendance
- Performs other duties as assigned.
Requirements:
SUPERVISION Directly reports to Chief Clinical Officer and works collaboratively with Department Directors to establish protocols and maintain program initiatives and integrity. Has indirect supervision from physicians and mid-level providers.
MINIMUM QUALIFICATIIONS
Education: Master’s degree in social work; License as a Clinical Social Worker (LCSW) required.
Experience: Minimum of one or more years preferred
Environmental Conditions: Medical Office environment
Physical Conditions:
- Must be able to work as scheduled – typically from 8:00 – 5:00 M-F
- Must be able to sit and/or stand for prolonged periods of time
- Must be able to bend, stoop and stretch
- Must be able to lift and move boxes and other items weighing up to 30 pounds.
- Requires eye-hand coordination and manual dexterity sufficient to operate office equipment, etc.
