Job Overview
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team at Wabash Senior Living and Rehab. The ideal candidate will possess a strong foundation in physiotherapy practices, with experience across inpatient, outpatient, and home health settings. As a Physical Therapist, you will be responsible for developing personalized care plans, delivering effective therapy services, and collaborating with multidisciplinary teams to promote patient recovery and well-being.
Key Duties & Responsibilities
- Patient Assessment: Assess patients’ physical conditions through comprehensive evaluations, utilizing your knowledge of physiology, anatomy, and medical terminology.
- Care Planning: Design and implement individualized treatment plans tailored to each patient’s unique needs, including discharge planning and progress monitoring.
- Versatile Care Delivery: Provide therapy services across various settings such as hospitals, clinics, outpatient centers, and home health environments.
- Advanced Modalities: Utilize targeted techniques, such as trigger point therapy and other physiotherapy modalities, to facilitate pain relief and mobility improvement.
- Education: Educate patients and their families on targeted exercises, injury prevention strategies, and home care routines to support ongoing recovery.
- Documentation: Document patient progress accurately within electronic medical records (EMR systems) while ensuring strict compliance with healthcare regulations.
- Collaboration: Partner with physicians, nurses, and other healthcare professionals to optimize patient outcomes across acute care, geriatrics, pediatrics, orthopedics, and community care contexts.
