Job Summary:
We are seeking a highly motivated and experienced RN Registered Nurse Home Health Clinical Director to join our team at Deer Meadows Home Health in Philadelphia, Pennsylvania. As a RN Registered Nurse Home Health Clinical Director, you will be responsible for overseeing the clinical operations of our home health department, ensuring high-quality patient care and compliance with regulatory requirements.
About the Home Health Clinical Director Role:
As a RN Registered Nurse Home Health Clinical Director, you will be responsible for:
Leading and directing a team of home health the agency clinical leadership (RN Clinical Manager & DOR), ensuring effective communication and collaboration to achieve departmental goals and objectives
Providing clinical guidance and mentorship to staff members, promoting evidence-based practice and continuous quality improvement
Developing and implementing patient care plans and programs, ensuring compliance with regulatory requirements and accreditation standards
Facilitating interprofessional collaborations with other departments, including rehabilitation, therapy, and social work services
Analyzing clinical data and metrics to identify trends and opportunities for improvement
Home Health Clinical Director Responsibilities:
Supervise and coordinate the activities of the home health department, ensuring efficient and effective use of resources
Develop and maintain relationships with referring physicians, patients, and families to ensure high-quality patient care and satisfaction
Collaborate with other departments to develop and implement education and training programs for staff members
Conduct ongoing clinical evaluations and assessments of patient care, identifying areas for improvement and implementing corrective actions
Maintain accurate and up-to-date records of patient care, including electronic medical records and departmental data
Home Health Clinical Director Requirements:
Current RN license in the state of Pennsylvania
Minimum 2-3 years of experience in home health or related field, with at least 1-2 years in a leadership or management role
Strong clinical knowledge and experience in home health care, with ability to adapt to changing regulations and standards
Excellent communication, leadership, and organizational skills, with ability to work effectively in a fast-paced environment
