Conduct comprehensive assessments of patients' physical and mental health, substance use history, and social determinants of health.
Develop individualized care plans based on patients' needs and treatment goals, integrating behavioral health, addiction medicine, and primary care interventions.
Administer medications, provide medication-assisted treatment (MAT) for substance use disorders, and monitor patients' responses to treatment.
Collaborate with an interdisciplinary team, including physicians, therapists, social workers, and other healthcare professionals, to ensure coordinated and holistic care.
Offer evidence-based counseling and psychoeducation to patients and their families, focusing on mental health, addiction, and chronic disease management.
Conduct regular follow-up visits to monitor patients' progress, provide ongoing support, and adjust treatment plans as needed.
Maintain accurate and up-to-date medical records and documentation, adhering to regulatory guidelines and best practices.
Graduation from an accredited school of nursing meeting minimum standards of the Florida State Board of Nursing as a Licensed Practical Nurse.
Experience:
Minimum of one (1) year experience in nursing, preferably in a mental health setting.
Pay: $20.00 - $25.00 per hour
Expected hours: 40 per week
