Hospice Care Team is looking to hire a RN Case Manager to assist with our hospice patient visits throughout the Golden Triangle. Working on a PRN basis, the nurse will work with a team of healthcare providers to ensure our hospice patients are receiving quality care. Pediatric experience is a plus!
Responsibilities:
- Conducting routine visits, admissions visits and/or follow-up visits at home or other facilities settings.
- Coordinating, assessing, and implementing patients' care.
- Providing accurate and timely documentation of patient services to reflect plans of care.
- Flexibility to be available days, evenings and weekends.
Qualifications:
- RN licensure (current, unrestricted) in the State of Texas
- Hospice experience
