Family Friends Home Health Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing home health care team. The LPN will provide skilled nursing services to clients in their homes, helping individuals maintain independence, safety, and quality of life.
This position offers the opportunity to build meaningful relationships with clients while providing personalized care in a home setting.
Responsibilities:
- Provide skilled nursing services to clients in accordance with physician orders and care plans
- Complete nursing assessments, medication management, and ongoing monitoring of client conditions
- Perform treatments, wound care, and other nursing tasks as ordered
- Monitor and report changes in client health status to the supervising RN/clinical team
- Educate clients and families on health care needs, medications, and disease management
- Accurately document all visits, care provided, and client updates
- Collaborate with caregivers, family members, and healthcare professionals to ensure quality care
- Maintain compliance with Indiana home health care regulations, HIPAA, and agency policies
Qualifications:
- Current and active Indiana LPN license
- Previous home health, long-term care, hospice, or clinical nursing experience preferred
- Strong communication and organizational skills
- Ability to work independently and manage patient care in the home setting
- Reliable transportation and willingness to travel locally
