- Performing duties as outlined in the plan of care.
- Effectively communicating updates and changes to patient's status to the supervising RN.
- Maintaining a safe patient environment.

Providing emotional support and compassionate care to patients and families.

Maintaining current patient records and progress notes.
- Working as a member of the interdisciplinary team.
- Current CNA certification.

At least 18 years of age.
- High school diploma or GED.

Valid driver's license, car insurance and reliable transportation.
- Willingness to travel throughout assigned service area.
- Can push, lift or carry up to 50 lbs. and can climb, balance, kneel and crouch.
- Comprehensive training including patient visits with a preceptor and Brightree and Elsevier training.
