We are growing and seeking a motivated Community Liaison to help expand our presence in the community and strengthen relationships with referral partners.
Community Liaison – Home Health Representative Job Summary
The Community Liaison is responsible for promoting the agency’s home health services, developing referral relationships, and identifying community needs. This role acts as the primary link between the agency, healthcare professionals, and community organizations. **This role will cover Greenville, TX and Sherman, TX**
Community Liaison – Home Health Representative Responsibilities
- Build and maintain strong relationships with physicians, hospitals, senior living communities, and other referral sources.
- Promote home health services to increase awareness and drive patient referrals.
- Assess community needs and communicate opportunities to agency leadership.
- Assist with development of new programs and policies related to service growth.
- Ensure all marketing activities comply with local, state, and federal regulations.
- Represent the agency at community events, health fairs, and professional networking opportunities.
- Collaborate with administrative and clinical teams to achieve agency goals.
- Perform additional duties as required by leadership.
Community Liaison – Home Health Representative Minimum Qualifications
- Bachelor’s degree preferred (Healthcare, Marketing, Communications, Business, or related field).
- Previous experience in healthcare, home health, hospice, senior living, or related field required.
- Solid referral sources and a proven track record for exceeding goals strongly preferred
- Excellent communication and interpersonal skills.
- Strong relationship‑building and customer‑service abilities.
- Ability to work independently and manage a flexible schedule.
- Professional, organized, and goal‑driven approach.
Work Environment & Requirements
- Requires local travel to referral sources and community locations.
- Must maintain reliable transportation.
- Ability to work in various environments including facilities, offices, and patient‑related settings.
- Ability to handle stressful situations calmly and professionally.

Pay: $65,000.00 - $85,000.00 per year
