Job Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will collaborate with a multidisciplinary team to develop and implement individualized care plans while promoting patient independence and quality of life.
Duties
- Conduct comprehensive assessments of patients' health status and needs in their home environment.
- Administer medications and treatments as prescribed, including IV insertion and catheter care.
- Utilize electronic health record systems such as eClinicalWorks and Epic to document patient information accurately.
- Educate patients and their families on health management, medication administration, and disease prevention.
- Collaborate with physicians, nursing homes, and other healthcare professionals to coordinate patient care.
- Monitor patients for changes in condition and respond appropriately to emergencies or complications.
- Stay updated on ICD-9 and ICD-10 coding for accurate documentation of diagnoses and procedures.
- Provide support in trauma medicine situations when necessary.
Experience
Candidates should possess the following qualifications:
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in home health care or a teaching hospital setting is preferred.
- Familiarity with electronic health record systems such as eClinicalWorks or Epic is a plus.
- Strong clinical skills, particularly in IV insertion and catheter care.
- Excellent communication skills for effective patient education and collaboration with healthcare teams.
- Ability to work independently while managing multiple patient cases efficiently.
