About the Position
We are looking for a dedicated and compassionate Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Minnesota. This part-time role combines in-home patient visits with remote documentation, care coordination, and follow-up activities, offering flexibility and meaningful one-on-one patient care.
Responsibilities
- Conduct in-home patient assessments and nursing visits
- Develop, implement, and monitor individualized care plans
- Administer medications and treatments as ordered by physicians
- Provide patient and caregiver education
- Monitor patient progress and report changes in condition
- Coordinate care with physicians, therapists, and other healthcare providers
- Complete clinical documentation and charting remotely
- Participate in virtual care planning and team meetings
Qualifications
- Current, active Minnesota Registered Nurse (RN) license
- Home health, hospice, case management, or clinical nursing experience preferred
- Strong assessment and critical-thinking skills
- Excellent communication and organizational abilities
- Proficiency with electronic health records (EHRs)
- Reliable transportation and valid driver's license for home visits
