Duties
- Conduct comprehensive evaluations of patients’ physical conditions, including mobility, strength, and functional abilities.
- Develop individualized treatment plans utilizing techniques such as trigger point therapy, orthopedics, neurology, geriatrics, and outpatient rehabilitation strategies.
- Implement therapeutic interventions to promote recovery, including manual therapy, exercise programs, and patient education.
- Collaborate closely with healthcare professionals such as physicians, nurses, and occupational therapists to coordinate care plans and optimize patient outcomes.
- Manage clinical documentation standards by accurately recording treatment sessions, progress notes, discharge planning details, and compliance with electronic health records (EHR) systems.
- Assist in discharge planning by evaluating patient progress and providing recommendations for continued care.
- Work with diverse patient populations including geriatric patients, post-surgical individuals, and those requiring outpatient rehabilitation services.
Skills
- Extensive knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques necessary for effective treatment.
- Proven experience in outpatient clinic settings, home health care, or outpatient rehabilitation centers.
- Strong ability to develop comprehensive care plans tailored to individual patient needs across various specialties like orthopedics and neurology.
- Excellent client communication skills to educate patients on treatment procedures and motivate active participation in their recovery journey.
- Familiarity with clinical documentation standards and proficiency in EMR (Electronic Medical Records).
- Ability to work collaboratively with multidisciplinary teams to ensure seamless patient care transitions from acute care to community-based services.
- Experienced in working with patients of diverse ages, backgrounds, abilities, and rehabilitation needs in a physical therapy setting.
