Ultimate Therapy is seeking a PRN Physical Therapist (PT) to support our skilled nursing rehabilitation team.
Primary Responsibilities:

Conduct physical therapy evaluations and establish treatment plans

Provide therapy focused on mobility, strengthening, balance, endurance, and pain management

Collaborate with PTA staff and interdisciplinary care teams

Monitor resident progress and maintain accurate documentation
Qualifications:

Active PT license required
-
Skilled nursing experience preferred but not required
