Quality Healthcare Group Inc. is seeking a licensed Physical Therapist to provide skilled, evidence-based therapy services to adults with medical, neurological, and/or intellectual/developmental needs in home and residential settings. The PT will evaluate functional mobility, design individualized treatment plans, and collaborate with interdisciplinary staff to maximize independence, safety, and quality of life.

Evaluation & Plan of Care
Perform initial comprehensive PT evaluation including functional mobility, fall risk, strength, balance, pain, safety, and home environment assessment.
Establish measurable functional goals and a time-limited plan of care consistent with physician orders.
Ensure therapy services meet skilled need criteria (clinical judgment required, not safely performable by untrained personnel).
Participate in OASIS data collection when required by agency policy.
Submit plan of care for physician certification and recertification as required.
Skilled Intervention
Provide evidence-based skilled therapy such as:
gait training and transfer training
therapeutic exercise and neuromuscular re-education
balance and fall-prevention interventions
pain management and mobility restoration
prosthetic/orthotic and assistive device training
Instruct patient and caregivers in home exercise programs and safety strategies, documenting patient response and carryover.
Reassessment & Medical Necessity
Conduct periodic reassessments per Medicare timing rules.
Document progress toward functional goals and justify continuation or discharge.
Identify when services are no longer skilled or medically necessary and initiate discharge planning.
Documentation & Compliance
Complete timely, defensible documentation supporting:
skilled need
homebound status (when applicable)
functional improvement or maintenance rationale
objective outcome measures
Maintain compliance with:
Participate in case conferences and interdisciplinary coordination.
Required Qualifications
Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.
Active Pennsylvania Physical Therapist license in good standing.
CPR certification.
Valid driver’s license and reliable transportation for community visits.
Ability to work independently in home/community environments.
Experience with Axxess or AloraPLUS
Experience in home health, geriatrics, neuro rehab, or IDD/TBI populations.
Prior experience training caregivers or residential staff.
Bilingual skills a plus.
