Overview
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team on a PRN basis. The ideal candidate will possess a strong clinical background, excellent patient care skills, and the ability to adapt to diverse healthcare environments. As a Licensed Vocational Nurse - PRN, you will provide vital nursing support across various settings, ensuring high-quality care for patients with complex medical needs. This role offers flexibility and the opportunity to work in dynamic healthcare environments, including long-term care, acute care, and community-based settings.
Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using HCHB.
- Provide specialized care for patients with Alzheimer's disease, dementia, or those requiring memory care and behavioral health support.
- Manage acute pain and respiratory conditions, including airway management and ventilator support when necessary.
- Collaborate with interdisciplinary teams to develop discharge planning, case management strategies, and patient education tailored to individual needs.
- Ensure infection control standards are maintained through aseptic techniques and proper sterilization procedures.
- Document all patient interactions meticulously in medical records while adhering to ICD coding standards like ICD-9 and ICD-10 for accurate billing and reporting.
Skills.
- Proficiency in EMR/EHR systems such as HCHB.
- Ability to work effectively with individuals with disabilities or developmental delays; familiarity with dementia or Alzheimer's care is highly valued.
- Excellent communication skills for patient education and team collaboration; physical ability to assist with mobility aids like Hoyer lifts when necessary.
