CAE RN- Hybrid Schedule – Full Time - $2,500 Staged hiring bonus!

Seeking a full time (37.5 hours/wk) CAE RN nurse to work within ESWA’s Nursing Department. The CAE RN applies MassHealth regulation 130 CMR 456.409 to conduct on site clinical eligibility screenings at area nursing facilities while also collaborating with ESWA’s Community Transition Liaison Program (CTLP), nursing facility, and other community agencies on discharge planning, as needed. The screening types include conversions, retrospectives, short term reviews, nursing facility transfers and continued stay. Completes Acute Inpatient Hospital and Non-Acute hospital-based screenings for MassHealth recipients or applicants seeking a pre-admission screening. Maintains appropriate paper and electronic files in accordance with Executive Office of Aging and Independence (AGE) and MassHealth regulations and standards.
QUALIFICATIONS/ABILITIES:
1. A valid Massachusetts license to practice as a Registered Nurse required.
2. A Bachelor of Science degree in Nursing or an Associates of Science degree in Nursing and at least one (1) year of clinical experience and/or nursing in a community or long-term care setting or a Diploma RN and two (2) additional years of community-based nursing experience or nursing facility experience required.
3. Knowledge of nursing facility discharge planning preferred.
4. Use of a reliable vehicle including active vehicle insurance with a current license to drive required.
5. Demonstrate strong organizational, prioritization, and problem-solving skills.
6. Ability to multi-task, keep accurate records, have a strong attention to detail, document clearly, and communicate effectively verbally and in writing, all within a fast-paced environment.
7. Ability to take initiative and work independently.
8. Must have moderate physical mobility. Ability to lift and carry objects weighing up to 20 pounds. Ability to climb stairs.
9. Ability to work in an interdisciplinary model.
10. Computer proficiency, including experience with Microsoft Office products (Excel, Word, SharePoint and Outlook)
· A staged hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year of active employment. The bonus for this position is $1,000.
