Overview:
We are seeking a dedicated Registered Nurse or LPN to join our Home Health team. The ideal candidate will provide compassionate and high-quality care to patients in the comfort of their homes.
We are currently looking to staff homes in the Lancaster, Ohio to treat total care individuals.
Duties:
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patient progress and update medical records
- Educate patients and their families on managing illnesses and injuries
- Collaborate with interdisciplinary healthcare teams to ensure comprehensive care
Qualifications:
- Bachelor's or Associate's degree in Nursing
- Valid RN/LPN license in the state of practice
- Experience in home health, primary care, or related field preferred
- Proficiency electronic medical records systems
- Strong understanding of medical office standards and procedures
- Knowledge of behavioral and occupational health
Nice-to-Have Skills:
- Experience working with Adults in an ALF healthcare setting
- Familiarity with primary care practices and protocols
