Position Summary:
Fairfax Rehab & Nursing Center is looking for a Unit Clerk-CNA to assists with resident admission and discharge, including paperwork and chart preparation.
Essential Duties and Responsibilities:
- Provides clerical support to Unit; answers the phone, faxes notifications and lab results to physicians, schedules appointments, checks unit mailbox and retrieves lab results from the printer.
- Assists in transporting residents within the facility as needed.
- Transcribes physician orders and completes the non-clinical portion of paperwork for new and re- admissions.
- Gathers information for the daily stand-up meeting (e.g. 24-hour report books care plan books and resident profiles).
- Schedules appointments.
- Files new information into medical records, updates meal percentage, weight sheets, etc.
- Informs manager of residents with decreased meal intake.
- Faxes dietary recommendations to the physician, documents weight and vital signs.
- Assists with admission and discharge of residents.
- Assist with pass trays at mealtimes.
- Prepares charts for new admissions; enters face sheet.
- Transports residents and charts to exam room for on-site visits by specialists.
- Assures that all call bells are answered promptly, meeting all residents’ needs in a timely manner.
- Reports incidents, accidents or changes in condition to the RN Supervisor immediately.
- Strictly adheres to infection control and safety rules, regulations, policies and procedures.
- Maintains the confidentiality of all resident care information, including protected health information in accordance with HIPAA and other federal and state regulations.
- Performs all tasks and duties in an efficient and safe manner.
- Performs other related duties as assigned or as necessary.
- Remains flexible and adaptable in work schedules and work assignments as defined by departmental and facility needs.

Qualifications:
Must have an active VA CNA license.
High School Diploma or equivalency, with office and computer experience preferred.
1-3 years’ experience in a long-term care setting preferred.
Previous administrative/secretarial experience preferred.
Must be able to effectively communicate with older adults in a manner that conveys warmth and professionalism.
