Pay Rate: $35.00 $38.00/hr
Contract: 13 Weeks
Schedule: Days
Guaranteed Hours: 36 hrs/week
Experience: Minimum 2 years home health experience required
EMR: Epic
Requirements: LPN License, BLS
About the Role
We are seeking an experienced Licensed Practical Nurse for a 13-week home health contract assignment supporting patients across the Vermont and New Hampshire service area. This role requires a compassionate and independent LPN who can provide patient-centered care in home-based settings while working closely with the registered nurse and interdisciplinary care team.
Responsibilities
Provide nursing care to patients in their residence
Perform delegated patient care under RN supervision
Assist with patient care plans and care updates
Observe and report changes in patient condition
Support patient comfort, safety, and daily care needs
Document patient care accurately in Epic
Collaborate with RNs, providers, patients, and families
Requirements
Active Licensed Practical Nurse license in Vermont and New Hampshire, or compact license required
Graduate of an approved practical nursing program
Current BLS certification required
Minimum 2 years of home health experience required
Strong patient assessment and documentation skills required
Must be comfortable working independently in home-based care settings
Excellent communication and patient-care coordination skills required
