Regional Wraparound Facilitator Supervisor

The Regional Wraparound Facilitator Supervisor will oversee wraparound facilitators through establishing a department-wide oversight and supervision of the service model which focuses on providing youth and family-centered supports who participate in the Children with Serious Emotional Disorders waiver (CSEDW) and WV Safe-at-Home (SAH) programs.
Job Duties:
1. Direct supervision of wraparound facilitators working in CSEDW and SAH.
2. Assist Director in hiring of new employees, yearly evaluations, disciplinary actions, and oversight of wraparound services.
3. Be initial point of contact for all Wraparound Facilitators.
4. Participate in an on-call rotation for your department.
5. Complete weekly fidelity supervision checks per NWIC (National Wraparound Implementation Center) model with each WF (wraparound facilitator) working in the CSEDW and SAH programs.
6. Hold departmental meetings every two weeks to delineate changes within the CSEDW and SAH for the NWIC programs to WFs.
7. Complete payroll for each Wraparound Facilitator for each pay period.
8. Maintain a caseload of five members at all times.
9. Meet with each Wraparound Facilitator monthly to discuss current caseload and any needs that may be present.
10. Collaborate with Director in assigning caseloads to WFs.
11. Ensure proficiency with new hires through monitoring completion of required NWIC/CSEDW/SAH/ onboarding trainings for WFs.
12. Ensure all paperwork for therapeutic foster care visits is completed monthly per home visit requirements for CSEDW.
13. Monitor billing/productivity of WFs working in CSEDW and SAH to ensure services are being rendered as designed and records are accurate for audits/reviews with Acentra/DARTS fidelity standards.
14. Assist with preparation for audits by reviewing member files and other documentation. (Acentra, DART, etc.)
15. Assist assigned WFs in meeting required assessments/documents for 30-day master plans.
16. Ensure department required trainings are renewed yearly for WFs (CANS).
17. Conduct department training on CANS assessment as part of wraparound facilitator model ongoing progress monitoring.
18. Review and submit all POCs submitted by WFs to Aetna, as well as review and submit edits, all within the required timeline (14 days for initial submission, 2 days for edits).
19. Ensure all WFs disseminate copies of the POC, CANS, transfer papers, discharge papers to CFT team members within 14 days of the meeting date.
20. Comply with all company/state/federal reporting requirements.
21. Ensure Significant life event meetings are facilitated within 48 hours of a critical incident report or grievance family may have with therapeutic services.
22. Travel as needed to statewide locations to train new Wraparound Facilitators.
23. Sample audit records and billing for quality measures and compliance.
24. Other duties assigned by direct supervisors for effective management and delivery of Wraparound services.
25. Will work at least two days in the office per week and collaborate with Director.
Qualifications:
1. Hold an WV current/active LSW plus 2 years or have a bachelor’s degree plus 5-years’ experience working with individuals with emotional or behavior disorders or foster care system
2. Non-LSW’s must hold a current conflict-free case management certificate from DHHR
3. CANS and/or CAFAS assessment certification
4. Knowledge of WRAP model through WV Foster Care model
5. Excellent verbal and written communication skills.
6. Ability to travel between locations (valid driver’s license, registration, and automobile insurance).
7. Ability to multi-task and mediate meetings
8. Familiarity with Positive Behavior Support implementation model
9. CPR/FA certification
10. Complete of all CSEDW/SAH required training pertaining to licensure
