The Department of Family Services (DFS) is seeking a Public Health Nurse II to support older adults, adults with disabilities, children, and caregivers through assessment, case management, care coordination, and Long-Term Services and Supports (LTSS) screening. This position conducts home and field visits, evaluates client health and safety needs, determines eligibility for Medicaid-funded LTSS programs, develops care plans, coordinates services, and provides health education and crisis intervention.
The Public Health Nurse II works collaboratively with multidisciplinary teams while maintaining a person-centered and culturally responsive approach. Responsibilities include managing a caseload, completing comprehensive assessments, making referrals to community resources, conducting outreach and health promotion activities, participating in trainings and meetings, and mentoring staff as needed.

Graduation from an accredited nursing program.

Current licensure as a Registered Nurse (RN) in the Commonwealth of Virginia, or eligibility to obtain Virginia licensure prior to employment.

Experience in public health nursing, case management, care coordination, long-term care, geriatrics, disability services, community health, or a related nursing field.

Ability to conduct home and field visits and assess clients with complex health, functional, and social needs.

Knowledge of nursing principles, health assessment, care planning, community resources, and applicable federal, state, and local regulations.

Strong communication, critical thinking, documentation, and interpersonal skills.

Valid driver's license and ability to travel throughout the community to conduct client visits.

Experience completing LTSS screenings and Virginia Uniform Assessment Instrument (UAI) assessments.

Experience working with older adults, adults with disabilities, and vulnerable populations.

Knowledge of Medicaid-funded long-term services and supports programs and community-based resources.

Bilingual language skills are a plus.
