EXAMPLE OF DUTIES
This position supports individuals by coordinating services, promoting independence, and ensuring their needs are understood and met. As a County Caseworker 1, you will perform the following duties:

Service Planning: Create individualized service plans that reflect each person's needs and goals

Resource Coordination: Connect individuals with providers, community supports, and natural supports that fit their needs

Monitoring Supports: Check that services are delivered properly and match what is outlined in the service plan

Health And Safety: Promote well-being by addressing safety concerns and supporting healthy choices

Person Centered Support: Encourage individuality, choice, and control in the lives of the people you assist
Work Schedule and Additional Information:

Full-time employment.

Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
