QUALIFICATIONS:
Must have experience with patient transportation. Excellent driver with clean driving record.
Licensing/Certification: Current Louisiana Driver’s License
SPECIAL DEMANDS :
Ability to lift up to fifty pounds and operate vehicle and equipment for special needs and wheelchair residents

DUTIES AND RESPONSIBILITIES :
- Transport residents to various places (usually to physician’s appointments).
- Assist resident into location, wait in lobby and return to nursing home safely, in a timely manner.
- Van Check is needed before resident arrives at the automobile. Check windshield, tires, and safety items. In extremely cold or hot weather, warm up vehicle ahead of time to provide a comfortable temperature for the resident at the time of departure.
- Fill out travel log.
- When van is ready, collect resident and aid for the trip. If an aide is not traveling with you, obtain any necessary medical information on the resident and take any paperwork with you. Make sure you have the correct address, time of appointment and etc. with you on the trip.
- Carefully assist resident on board the van. Special care should be made to insure safe boarding. Make sure seat belts and other belts are secured and wheelchair’s brakes are locked. Making the resident feel comfortable during this transition time will give a more positive trip.
- If the radio is played during the trip the music should be appropriate for the resident and at a sound level where the resident’s voice can easily be heard by the driver.
- Do not eat, drink, smoke or operate a cell phone in the vehicle while transporting residents. If a call must be answered, an employee is expected to pull safely and legally off the road and accept or return the call.
After return:
- Return resident safely to the nurse’s station along with any necessary paper work and belongings that the resident may have taken with them
- Return to the van, complete travel log and pick up any trash left in the van.
- If during the trip anything was noted that maintenance needs to be made aware of, fill out the proper work order and turn it in with the keys. Gas card and receipts should be turned in at the same time.
OTHER WORK VARIABLES :
Working Hours: Monday - Friday and occasionally Saturday. Shift Hours Vary, but are always during day time and evening hours. No night shifts.
