Position Summary
We are seeking a PRN / Part-Time Physical Therapist Assistant (PTA) to provide home-based physical therapy visits to patients throughout LaPorte County, Indiana. This role offers flexible, visit-based scheduling — ideal for a PTA looking to supplement their caseload or build a flexible home care schedule while working under the direction of a licensed Physical Therapist.
Key Responsibilities
· Deliver physical therapy treatment plans established by the supervising Physical Therapist in patients' homes
· Assist patients with therapeutic exercises, gait training, balance work, and mobility techniques
· Monitor and document patient progress, reporting changes in condition to the PT and care team
· Educate patients and caregivers on home exercise programs and safety techniques
· Travel to patient homes throughout LaPorte County, Indiana
· Maintain timely, accurate clinical documentation in compliance with home health regulations
· Communicate effectively with the interdisciplinary care team
Qualifications
· Current Indiana PTA license (required)
· Prior home health experience preferred
· Valid driver's license and reliable transportation
· Strong communication and organizational skills
· Ability to work independently while following PT-directed care plans
· Comfortable managing a flexible, visit-based schedule
What We Offer
