Job Summary
Licensed Practical Nurse provides basic nursing care, including vital signs, medication administration, wound care, and assisting residents with daily activities, under the supervision of a Registered Nurse (RN)
support and comfort to residents and their families.
Essential Duties and Job Responsibilities

Basic Nursing Care:
-
Vital Signs: Accurately measure and record vital signs like temperature, pulse, blood pressure, and respirations.

Medication Administration: Administer medications as prescribed by a physician, following facility protocols and under the supervision of an RN.

Wound Care: Perform wound care, including changing dressings and monitoring for signs of infection.

Hygiene and Mobility: Assist residents with personal hygiene, dressing, bathing, and mobility.

Nutrition and Hydration: Ensure residents receive proper nutrition and hydration, assisting with feeding if necessary.
Patient Monitoring:

Patient History: Record a patient's history and current condition.

Observation: Observe and document changes in a resident's condition, reporting any concerns to the RN.

Communication: Communicate effectively with residents, families, and other healthcare professionals.
Collaboration and Supervision:

RN Supervision: Work under the supervision of an RN, following their guidance and instructions.

CNA Supervision: Supervise certified nursing assistants (CNAs) in providing basic nursing care.

Teamwork: Collaborate with other healthcare professionals to ensure residents receive comprehensive care.
Other Duties:

Patient Safety: Maintain a safe environment for residents, following infection control protocols and other safety guidelines.

Documentation: Accurately document all care provided, including medications administered, vital signs, and any observations.

Emotional Support: Provide emotional support and comfort to residents and their families.
Requirements
-
Must possess a nursing degree from an accredited college or university.
- Must possess a current, unencumbered, active license to practice as an LPN in this state.

Must maintain and have an active CPR/BLS during employment.

Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting.
Experience in electronic medical records documentation.
