Job Summary
We are seeking a dedicated and skilled Home Health Pediatric Physical Therapist to join our healthcare team. The ideal candidate will be responsible for evaluating and treating patients with various physical conditions, developing individualized care plans, and implementing therapeutic interventions to enhance patient recovery and mobility. This role requires a comprehensive understanding of anatomy, physiology, and medical terminology, as well as experience in both inpatient and outpatient settings.
Duties
- Conduct thorough evaluations of patients to assess their physical abilities and limitations.
- Develop and implement personalized care plans tailored to each patient's needs.
- Provide hands-on physiotherapy treatments including trigger point therapy and orthopedic rehabilitation.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.
- Educate patients and their families about treatment plans, exercises, and home care strategies.
- Document patient progress accurately using EMR systems.
- Participate in discharge planning to facilitate smooth transitions from hospital to home or community care.
- Maintain up-to-date knowledge of best practices in physical therapy, including acute care, geriatrics, pediatrics, and outpatient services.
Requirements
- A degree in Physical Therapy from an accredited institution.
- Valid state license to practice as a Physical Therapist.
- Strong knowledge of anatomy and physiology relevant to patient care.
- Experience in home health, community care, inpatient settings, or clinic environments is preferred.
- Familiarity with medical terminology and EMR systems is essential.
- Excellent communication skills for effective patient interaction and collaboration with healthcare teams.
- Ability to develop comprehensive care plans that address diverse patient needs across various age groups including geriatrics and pediatrics.
