We are seeking a compassionate, skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our clinical team.
Responsibilities:
- Conduct comprehensive physical exams, preventive health screenings, and acute sick visits for adult patients.
- Manage chronic conditions (e.g., hypertension, diabetes, thyroid disorders) with tailored long-term care plans.
- Order, perform, and interpret diagnostic lab tests and diagnostic imaging.
- Prescribe medications, order therapies, and recommend health/wellness modifications in accordance with state guidelines.
- Educate patients on preventive care, lifestyle medicine, and disease management.
- Maintain complete, accurate, and timely electronic health record (EHR) documentation.
- Collaborate effectively with administrative and clinical staff to ensure seamless patient care.
Qualifications:
- License/Certification:
- Active, unencumbered Indiana NP or PA license (Required)
- Board certification via AANP, ANCC, or NCCPA (Required)
- Active DEA registration and Indiana CSR (or eligible to obtain before start) (Required)
- Certified Medical Examiner on the National Registry of Certified Medical Examiners (NRCME / DOT Certification) (Required or willingness to obtain within 60 days of hire)
- Current BLS/ACLS certification (Required)
- Experience: 1+ years of outpatient primary care or internal medicine experience preferred
Pay: $87,932.31 - $105,896.98 per year
