Responsibilities
- Assist clients with Activities of Daily Living (ADLs)
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care plans
- following HIPAA guidelines and facility protocols
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
- Maintain a clean and safe environment through regular cleaning duties and infection control practices
- Document patient observations, care activities, and communication with healthcare teams clearly and accurately
Qualifications
- Prior experience working in group homes, assisted living, nursing homes, or home health care settings is highly preferred
- Knowledge of patient observation technique.
- Familiarity with care plans, \ vital signs monitoring, and first aid/CPR certification is essential
- Strong communication skills for effective client interaction and team collaboration
- Commitment to maintaining HIPAA compliance and respecting client confidentiality
