Community Home Health Care, located in Chambersburg, is currently seeking a Medical Social Worker for performing home care visits in Waynesboro, Greencastle and close surrounding areas.

GENERAL DESCRIPTION OF RESPONSIBILITIES:
- Assess the patient and family with personal environmental difficulty in order to estimate their capacity and potential to cope with problems of daily living
- Participates in the development of and makes suggestions to the physician for additions to Plan of Care
- Provides supportive casework to assist in restoring the patient to his/her optimum level of social health adjustment
- Works with family to assist in utilization of appropriate community resources or to live within the disability
- Participates in discharge planning and in-service programs
- Acts as a consultant with other members of the team
- Assists clients and family in understanding, accepting, and following medical recommendations and provide services planned to restore the patient to optimum social and health adjustments within patient's capacity.
- Observes, records and reports social and emotional changes
- Provides liaison contact and follow up with other community agencies
- Coordinates referrals to other community agencies as needed
- Submits assessments and progress notes in a timely manner, as required by Agency policy
- Tracks our current patients in hospitals, rehabs or nursing homes and compiles a "hospital list."
CRITICAL DEMANDS:
- Visual/hearing ability sufficient to comprehend written/verbal communication
- Ability to effectively deal with stress
- Must be proficient with computer programs and texting
QUALIFICATIONS:
- MSW OR BSW
- Licensure by the State, if applicable
- Must complete or have completed a criminal history background check as required by Public Act 169 or 1996
- CPR Certification
- Valid Drivers License/Car Insurance
- 2 step TB test to begin
- Social Work in a healthcare setting preferred
Job Type: Part-time
Pay: $20.00 - $50.00 per hour
