Duties
- Deliver comprehensive nursing assessments, including vital signs, physical examinations, and patient history evaluations to determine individualized care plans.
- Administer medications, injections, IV infusions, and treatments such as tube feeding or catheter care while adhering to all safety protocols and HIPAA regulations.
- Monitor patients’ health status using advanced EMR (Electronic Medical Records) systems like Epic or Cerner to document medical records accurately and efficiently.
- Manage complex cases involving dementia care, Alzheimer's care, memory care, and patients with developmental disabilities or behavioral health needs.
- Provide specialized interventions such as wound care, IV insertion, sterile processing techniques, and acute pain management for diverse patient populations.
- Collaborate with physicians, case managers, and other healthcare professionals to coordinate discharge planning, post-acute care, or hospice services.
- Educate patients and families on health coaching topics including medication administration, nutrition, infection control, and disease management strategies.
Qualifications
- Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN).
- Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable.
- Demonstrated proficiency with EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks for accurate documentation.
- Experience working with individuals with disabilities or chronic illnesses including Alzheimer’s disease and dementia is preferred.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and diagnostic evaluation techniques.
- Ability to perform physical examinations, venipuncture (blood draws), phlebotomy, and sterile procedures confidently.
- Excellent patient assessment skills combined with compassionate communication to support diverse populations including pediatrics and geriatrics.
