Clinical Social Worker- Inpatient Rehab
Education and Licensure
Required:
Master’s degree in social work (MSW) from a CSWE-accredited program required.
Current unrestricted Kentucky Licensed Clinical Social Worker (LCSW) or equivalent independent clinical social work license.
Preferred:
Minimum of five years of post-licensure clinical social work experience in acute care, inpatient rehabilitation, hospital case management, or related healthcare setting preferred. Demonstrated experience with psychosocial assessment, discharge planning, utilization review, insurance authorization, and interdisciplinary care coordination. Working knowledge of CMS IRF regulations, Medicare reimbursement, post-acute care services, and payer requirements. Strong clinical assessment, crisis intervention, communication, organizational, and leadership skills.
The Clinical Social Worker is a master's-level licensed clinician responsible for providing comprehensive psychosocial services, utilization review, case management, discharge planning, and clinical leadership within the Inpatient Rehabilitation (IPR) program. This position provides direct patient and family intervention while coordinating interdisciplinary care to ensure medically necessary, patient-centered rehabilitation from admission through discharge and community reintegration.
The Clinical Social Worker serves as a clinical resource for social work and case management staff, collaborates with physicians and the interdisciplinary rehabilitation team, ensures regulatory and payer compliance, and promotes high-quality patient outcomes for individuals recovering from stroke, spinal cord injury, brain injury, amputation, orthopedic procedures, and other medically complex conditions.
