We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Home Health Aide (HHA) to join our dynamic healthcare team. Responsibilities
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using proper manual handling techniques and equipment like Hoyer lifts.
- Monitor vital signs including blood pressure, pulse, respiration, and temperature; document findings accurately within electronic health record (EHR) systems.
- Support medication administration under supervision, ensuring adherence to HIPAA regulations and maintaining patient confidentiality.
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care strategies and behavioral management.
- Collaborate with nursing staff to implement care plans for patients with complex needs.
- Assist with medical procedures like catheter care when trained and authorized.
- Maintain a safe environment by practicing aseptic techniques for infection control; adhere to safe lifting practices during patient transfers.
- Communicate effectively with patients, families, and healthcare team members to ensure comprehensive patient service and support continuity of care.
- Provide companionship and personalized care while encouraging independence and self-care.
- Prepare meals and snacks. May include grocery shopping.
- Perform light housekeeping, including vacuuming, laundry, and linen changes.
- Assist with incontinence care.
- Assist with transportation needs
Skills
- Proven experience working with diverse populations including geriatrics, pediatrics, individuals with disabilities, or those requiring hospice or palliative care.
- Familiarity with EMR/EHR systems for accurate medical documentation.
- Knowledge of medical terminology, anatomy, physiology, and basic life support (CPR).
- Ability to perform heavy lifting (at least 20lbs) safely while maintaining proper body mechanics; skilled in patient transfer methods using equipment like Hoyer lifts.
- Strong caregiver communication skills with the ability to provide compassionate patient service in high-pressure situations.
- Certification in basic life support (BLS), HIPAA compliance knowledge, and familiarity with infection control protocols are required.
- MUST speak and understand English clearly with elderly clients and staff.
- Current and valid Certified Nursing Assistant (CNA) or Home Health Aide certification.
- Must be available to work on weekends, evenings, and holidays.
- Required to have reliable transportation.
- Require a valid driver’s license and insurance.
- Prior home health experience preferred
