Jmkay Healthcare Services, Inc. is seeking an experienced Designated Coordinator (DC) to oversee service coordination and case management for individuals receiving services in our 245D Community Residential Services (CRS) program.
We are specifically seeking a professional with strong experience completing resident assessments, writing Individual Service Plans (ISPs), developing person-centered care plans, conducting annual and semi-annual reviews, and maintaining regulatory compliance with Minnesota 245D requirements.
This position is ideal for someone with a strong clinical, social services, case management, or behavioral health background who understands documentation, service planning, and coordinating supports for individuals with mental health diagnoses, substance use disorders, and other support needs.
Essential Responsibilities Assessment & Care Planning
- Complete comprehensive resident assessments and reassessments.
- Develop, implement, and update Individual Service Plans (ISPs) and support plans.
- Conduct annual evaluations and progress reviews.
- Ensure all service plans are person-centered and compliant with Minnesota 245D regulations.
- Monitor resident progress toward goals and revise plans as needed.
- Maintain accurate and timely documentation.
Service Coordination
- Coordinate services with case managers, guardians, healthcare providers, therapists, and community resources.
- Participate in admission, discharge, and transition planning.
- Attend interdisciplinary meetings and advocate for resident needs.
- Ensure continuity of services and supports.
Compliance & Quality Assurance
- Maintain compliance with Minnesota 245D licensing standards.
- Review resident records to ensure documentation is complete and current.
- Assist with licensing reviews, audits, and corrective action plans.
- Monitor service delivery and quality outcomes.
- Maintain HIPAA compliance and confidentiality requirements.
Resident Support
- Support residents in achieving personal goals and increasing independence.
- Assist with behavioral support planning and crisis response when needed.
- Promote resident rights, dignity, and person-centered services.
Qualifications Required
- Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, Behavioral Sciences, or a related field; or meet Minnesota 245D Designated Coordinator qualifications.
- Minimum 2 years of experience completing assessments, service plans, care plans, or case management documentation.
- Experience working with individuals with mental health diagnoses, co-occurring disorders, developmental disabilities, or substance use disorders.
- Strong knowledge of Minnesota 245D regulations.
- Excellent documentation and organizational skills.
- Ability to manage a caseload and meet regulatory deadlines.
Preferred
- Experience in Community Residential Services (CRS), Adult Foster Care, or other 245D programs.
- Experience writing assessments, support plans, and person-centered plans.
- Experience coordinating services with counties, case managers, and guardians.
- Positive Supports training.

Pay: $22.00 - $25.00 per hour
