Overview
Home Advantage Healthcare is hiring Registered Nurses for full-time, part-time and PRN positions serving Marion, Lake, and Sumter Counties.
Our nurses provide skilled nursing care to patients in their homes while working closely with physicians/providers, therapists, and other members of the interdisciplinary care team. This position offers nurses the opportunity to use their clinical skills, work independently, and provide individualized patient care while being supported by an interdisciplinary team.
Responsibilities
- Perform comprehensive nursing assessments, and complete accurate, timely documentation in electronic medical record (EMR).
- Develop and implement individualized care plans in accordance with physician/provider orders.
- Provide wound care, medication management, disease process education, and other skilled nursing services in accordance with physician orders.
- Complete OASIS Start of Care, Recertification, Resumption of Care, and Discharge assessments as assigned.
- Monitor patient progress regularly, communicate changes in condition to physicians and the interdisciplinary care team.
- Maintain strict confidentiality of medical records and sensitive information in compliance with HIPAA regulations.
- Provide case management for assigned caseload, including coordinating care, oversight of LPN services and completion of supervisory visits as required, ensuring plan of care is followed.
Qualifications
- Current Florida Registered Nurse (RN) license
- Current CPR/BLS certification
- Home health experience preferred
- Strong clinical assessment, case management, communication, and documentation skills
- Knowledge of home health regulations and standards of care
- Ability to work independently and effectively manage an assigned caseload
- Reliable transportation and the ability to travel within assigned service areas
Compensation
- Paid per visit rates starting at $50.00, with rates varying based on visit type
