Cass County Senior Living & Rehabilitation is seeking an experienced and motivated Registered Nurse (RN) to join our clinical leadership team as our MDS Nurse / Infection Preventionist. This unique leadership role combines the responsibilities of both the MDS Nurse and Infection Preventionist, allowing you to play a vital role in resident care, reimbursement accuracy, regulatory compliance, and infection prevention. Your schedule will be divided between both areas of responsibility while collaborating closely with the interdisciplinary care team.
Complete and coordinate MDS assessments in accordance with state and federal regulations
Ensure accurate coding to support reimbursement and quality measures
Develop, update, and monitor individualized resident care plans
Collaborate with the interdisciplinary team to promote exceptional resident outcomes
Maintain accurate and timely clinical documentation
Lead the facility's Infection Prevention and Control Program
Conduct infection surveillance, tracking, investigations, and required reporting
Audit infection control practices throughout the facility
Educate staff on infection prevention standards and best practices
Monitor compliance during all shifts, including periodic off-shift observations
Ensure compliance with CDC, CMS, and Illinois Department of Public Health (IDPH) infection prevention guidelines
Current Illinois Registered Nurse (RN) license required
Previous MDS experience required
Infection Prevention experience preferred, but not required
Willingness to complete the CDC Infection Preventionist Training if not already certified
Strong assessment, organizational, communication, and leadership skills
Ability to work independently while collaborating effectively with an interdisciplinary team
Participation in the facility's management on-call rotation is required
