Resolve at Diamondback is seeking a compassionate and organized Case Manager to join our team in Phoenix, AZ. This role plays an important part in supporting patients throughout their stay by coordinating care, assisting with discharge planning, communicating with families, and helping ensure patients have access to the resources they need for successful outcomes.
We are looking for someone proactive, detail-oriented, and passionate about patient care and advocacy.
Coordinate patient care plans and discharge needs
Communicate with patients, families, providers, and interdisciplinary teams
Assist with discharge planning, referrals, and resource coordination
Maintain accurate patient documentation and care records
Collaborate with nursing, therapy, admissions, and administrative teams
Help ensure smooth transitions of care and follow up support
Support compliance with facility policies and healthcare regulations
Previous case management, discharge planning, social services, or healthcare coordination experience preferred
Behavioral health, rehabilitation, skilled nursing, or healthcare experience preferred
Strong communication and organizational skills
Ability to multitask and work in a fast paced environment
Experience with insurance authorizations or community resources is a plus
