Join our growing hospice team as a Hospice Admission RN, where you'll have the opportunity to guide patients and families through one of life's most important transitions while working alongside a supportive, team-oriented organization.
We are looking for a compassionate, organized, and self-motivated nurse who thrives in an independent role and values building meaningful relationships with patients, families, referral partners, and facility staff.
What You'll Do
As a Hospice Admission RN, you will:
- Complete hospice admission assessments in patients' homes, assisted living communities, skilled nursing facilities, and other care settings.
- Determine hospice eligibility using Medicare guidelines.
- Educate patients and families on hospice philosophy, services, medications, and what to expect throughout their hospice journey.
- Coordinate admissions with physicians, hospitals, referral sources, facilities, and the interdisciplinary hospice team.
- Develop and initiate individualized plans of care.
- Complete accurate and timely electronic documentation.
- Ensure a smooth, compassionate transition into hospice services.
- Participate in an on-call admission rotation, if applicable.
Qualifications
- Current Texas Registered Nurse (RN) license.
- Minimum of one year of RN experience required.
- Hospice, home health, palliative care, emergency department, ICU, or case management experience preferred.
- Excellent assessment, communication, and time management skills.
- Ability to work independently while collaborating with an interdisciplinary team.
- Valid driver's license, reliable transportation, and current auto insurance.
