We’re seeking a dependable, motivated individual to join a Self-Determination Community Living Supports (CLS) program providing one-on-one support to an adult with physical and developmental disabilities.
This is one client in one private home. Unlike facility or agency work, you’ll have the opportunity to build a consistent working relationship while helping the individual achieve meaningful goals at home and in the community.
Schedule
This position offers approximately 17.5–30 hours per week, depending on staffing needs, school schedules, and employee availability.
Regular Schedule Includes:
Three Assigned Weekday Shifts
Monday Evening/Overnight
Additional hours may become available during school breaks, summer months, weekends, and when coverage is needed.
Scheduling Expectations
This position requires flexibility.
Shift assignments are based on the individual’s care needs and staffing requirements.
Employees do not choose their own workdays or customize the schedule.

Primary Responsibilities
Your role will focus on helping the individual achieve Person-Centered Plan (PCP) goals by providing support with:
- Community outings and community inclusion
- Daily living skills and independence
- Standing programs using adaptive equipment
- Pacer-assisted gait training
- Therapeutic positioning and mobility activities
- Hoyer lift transfers
- Personal hygiene and toileting assistance
- Brief changes, including when in the community if necessary
- Meal preparation and feeding assistance
- Safety supervision
- Accurate documentation of services provided
Physical Requirements
This is a physically demanding position requiring applicants to safely perform essential job duties, including:
- Frequent lifting, bending, stooping, kneeling, and repositioning
- Hoyer lift transfers
- Assisting with standing and mobility programs
- Pushing and maneuvering adaptive equipment
- Providing hands-on physical assistance throughout the shift

Qualifications
- Dependable attendance

- Strong communication and organizational skills
- Comfortable working independently in a home setting
- Ability to safely perform the physical requirements of the position
- Experience as a DSP, CLS worker, caregiver, CNA, Home Health Aide, or related field is preferred, but not required.
Pay: $22.20 - $22.40 per hour
