The Case Manager/Service Coordinator plays a vital role in assisting individuals in accessing and coordinating services that support independence, community integration, health, and overall well-being. This position is responsible for coordinating, advocating for, and monitoring services identified within the participant’s Service Plan to ensure all needs are appropriately addressed.
Services may include Medicaid State Plan services, HCBS Waiver services, medical and behavioral health supports, educational and vocational services, housing resources, and other community-based supports. The Service Coordinator works collaboratively with participants, families, providers, and interdisciplinary teams to promote person-centered care and successful outcomes.

Qualifications
- Bachelor’s degree preferred; Associate degree with relevant experience considered
- Experience in human services, case management, behavioral health, healthcare, social services, or related field preferred
- Strong communication, organizational, and advocacy skills
- Ability to work independently and within a multidisciplinary team
- Reliable transportation required for community-based visits
