We are seeking a dedicated and passionate Primary Care Physician to join our dynamic team. The ideal candidate will be responsible for providing comprehensive medical services to patients, focusing on disease prevention, health promotion, and patient education. This is a full-time, permanent position, offering a competitive salary and benefits package. Responsibilities:
1. Provide comprehensive patient care, including consultations, evaluations, treatments, and preventive health measures within prescribed guidelines and instructions of the supervising physician.
2. Develop and implement patient management plans, recording patient history and physical assessment results.
3. Treat chronic health conditions and promote disease prevention.
4. Implement medication-assisted treatments and monitor patients' medication therapies and treatment plans.
5. Integrate behavioral health into patient care and work closely with mental health professionals to ensure patients receive holistic care.
6. Maintain accurate, detailed reports and records using EMR documentation.
7. Collaborate with other healthcare professionals to form a healthcare team for each patient.
8. Educate patients and their families about their conditions and what they can do to improve their health.
9. Stay up-to-date with advancements in healthcare technology and best practices in family medicine.
Qualifications:
1. Medical Degree (MD) or Doctor of Osteopathy (DO) from an accredited institution.
2. Board Certified or Board Eligible in Family Medicine.
3. Valid and unrestricted license to practice medicine in the state.
4. Minimum of 2 years of experience in a primary care role.
5. Experience in chronic disease management and preventive care for adult populations.
6. Proficiency in medication-assisted treatment and behavioral health integration.
7. Experience working with electronic medical records (EMR) documentation.
8. Excellent communication and interpersonal skills, with a focus on patient education.
9. Strong commitment to health promotion and disease prevention.
10. Ability to work in a team-oriented environment and collaborate with other healthcare professionals.
